32F, been absolutely exhausted for 2+ years and had brain fog so bad and felt pretty much depressed. Doctor checked my blood work - folate was low, homocysteine was high. Started me on folic acid supplements.

The frustrating part

Took folic acid religiously for 6 months. Nothing changed.

January (baseline):

Folate: 8.2 ng/mL (low-normal)
Homocysteine: 18.5 µmol/L (high - should be <15)

April (3 months on 800 mcg folic acid):

Folate: 9.1 ng/mL (barely moved)
Homocysteine: 17.8 µmol/L (still high)

Doctor doubled my dose to 1600 mcg.

July (6 months total):

Folate: 10.3 ng/mL (still barely improved)
Homocysteine: 17.2 µmol/L (STILL high)

Doctor said I must not be taking it consistently. I was furious - I'd been taking it every single day.

What changed everything

Got genetic testing done. Found out I have MTHFR C677T: TT (homozygous).

This means my body has only ~30% normal enzyme activity to convert folic acid into methylfolate - the active form my cells actually use. I was basically taking a supplement my body couldn't process.

It's like trying to use a key in a broken lock. The folic acid was there, but my body couldn't convert it to the form it needed.

New approach

Switched to supplements that bypass my genetic limitation:

Methylfolate 1000 mcg (the active form, no conversion needed)
Methylcobalamin (active B12) 1000 mcg (needed for the methylation pathway)
Vitamin B6 (P5P) 50 mg (helps lower homocysteine through alternate pathway)

Results after 3 months

October (3 months on methylfolate protocol):

Folate: 22.4 ng/mL (finally optimal!)
RBC Folate: 685 ng/mL (jumped from 315 - this shows cellular levels)
Homocysteine: 9.2 µmol/L (NORMAL for first time in years!)
B12: 782 pg/mL (optimal)
B6: 45 ng/mL (optimal)

But more importantly - how I feel:

Brain fog cleared by week 2
Energy back by month 1
No more afternoon crashes
Can actually focus at work again
Feel better than I have in 5 years

So yeah, turned out my body literally couldn't use the supplements I was taking. My homocysteine went from 18.5 down to 9.2 in just 3 months once I switched to the right form.

If you're taking folic acid and your homocysteine isn't budging, or if you have chronic fatigue that no one can explain, might be worth looking at your MTHFR genetics. About 10-15% of people have variants that mess with folate metabolism.

The TT genotype (what I have) is pretty common - especially in people with Northern European ancestry. There's also CT (one copy) which gives you ~65% enzyme activity, or CC (normal).

Your regular blood work won't show this. But when you combine your blood test results with genetic data, suddenly it all makes sense. For me, it explained 6 months of failed treatment and years of feeling like garbage.

Anyway, hope this helps someone avoid the frustration I went through.
